Registration Policies:

REFUND POLICY:

·         Refunds will not be issued for non-attendance of a scheduled class

·         If you withdraw your child from the program, you must notify Sam’s Soccer Jam no less than 24 hours before the 2nd class of the season and are responsible for all charges until you do so.

• Sam’s Soccer Jam guarantees a refund for the remaining classes (after a $15 administrative/insurance fee) before the 2th class of any season. No refunds thereafter.
• Sam’s Soccer Jam reserves the right to cancel a class due to low enrollment. In the event this happens, you may choose a different class, or receive a complete refund.

Inclement Weather

• Sam’s Soccer Jam has sole discretion to determine if a class should be canceled due to weather (such as heavy rain, snow, high winds, extreme temperature). We will attempt to contact you via email or text message in the event a class is called off due to weather. We will also post notice of a rained out class on the SamSoccerJam.com home page.
• If you are unsure about the status of a class, call our office at (832) 786-1968 .

MAKE-UP CLASSES & CANCELLATIONS:

•  There will be a maximum of two make up weeks due to inclement weather (extreme cold, heat, or rain).
•  Sam's Soccer Jam reserves the right to cancel any program due to insufficient enrollment. Upon cancellation, a full refund will be made.

Discount and Fees

o   A $25.00 administration fee will be applied to all returned checks.

o   There will be a $10.00 LATE FEE for registration after the second class.

o   A $15.00 administration fee will be applied to any refund issued.

o   Sam’s Soccer Jam is not responsible for lost or stolen checks

o   Multi-Child Discount – The oldest child In the family pays the standard fee for the child's session. Second, third, fourth, etc. Child/Children each receive a discount (5% each).

TRANSFER POLICY:

Transfers will be accommodated within the same season as long as there is room in the class.

ARRIVAL & DISMISSAL POLICY:

Parents/Caregivers must sign children in & out of programs that are open to the public. No child can be left unattended before class.

If your child is to be picked up by someone other than yourself,  please provide a note to the coach. Children will not be released to anyone unknown to the Sam’s Soccer Jam Coach.

PHOTO POLICY:

I do hereby consent and agree that Sam’s Soccer Jam has the right to take

photographs and video of my child and to use these on the company

website www.samsoccerjam.org and promotional materials without

compensation. Your child's name and identity will not be revealed. If

you would prefer we do not photograph your child, please notify us
